Credit Application Form
For all New Credit Account Applications, the applicant must FILL OUT IN FULL the following forms and return them to:
Mayflower Importer - 77a Metrolink Circuit, Campbellfield. 3061
1. Credit Account Application Form
2. Personal Guarantee Form
3. Signature Box Form (attachment which must be returned with all Credit Account Applications)
** Only FULLY completed Credit Account Application will be accepted. All applications will be verified by the Directors before acceptance.
